On the inner display
iPhone Duo shows each app in one of three ways depending on which SDK it was built with: letterboxed with black bars (iOS 26 or earlier), expanded with some empty space (iOS 27), or full screen (iOS 27.1 and later). We don't yet know which applies here.
Plague Inc. launched as an iPhone game in May 2012. Ndemic Creations says it added full iPad support in the 1.1 update days later, after which the game became the top paid iPad app in the US. iPhone Duo support is not confirmed yet.
In Split View
iPhone Duo is the first iPhone that can show two apps side by side, and every app takes part — including Plague Inc.. Open it on the inner display, then add a second app next to it. You can save the pair and come back to it later.
